Claims Procedures and Documentation
On the happening of any accident or illness which give rise to a claim under this insurance the Insured shall
- Notify the Insurance Company within 21 days stating the circumstances of the loss, damage, liability, injury, death or illness
- Notify the police if there is evidence of criminal or malicious act involved
- All medical reports certificates and information required by the Insurance Company shall be furnished at the Insured/Insured Person’s expense
Documents required for Medical Claim
- Original Claim Form.
- Medical Report.
- Original Medical Receipts.
- Photocopy of the Policy Schedule.
- Any other documents that may be required by the Insurance Company.
- Photocopy of the worker's work permit OR the Work Permit cancellation letter, if he/she has been repatriated.
